The Organization's Application to Sponsor a Specialty License Plate (Form MV115) lists the requirements. Once an organization's application is approved, which may take up to eight weeks, the sponsor must either:

Pay a fee of $4,000 to reimburse the department for the initial costs of producing the specialty license plate.
OR
In lieu of the fee, file with the department a minimum of 400 prepaid applications for the sponsoring organization's specialty license plate. The organization specialty license plate is then programmed for manufacture.
